Referrals

>Your specialist is also capable of faxing a Referral Request with all the necessary information

>At least 5 business days prior too the visit, your specialist should fax the Referral Request as they have all the information required as well as our fax number.​

Referral Information Required

• Your Name, Date of Birth, and Phone Number
• Insurance Company and Plan, and insurance ID number.
• The complete Name of the Doctor or Facility you plan to visit and the date of that appointment.
• ​The preferred method to submit this information is through the NFPA Patient Portal

We usually refer to physicians within our excellent network of specialists most of whom have been dealing with us for a number of years. This allows us to match you and your problem with the most compatible doctor. Our experience has shown these doctors to be expert, available and they communicate with us in a timely fashion. 

Most HMOs discourage completion and/or submittal of referral forms AFTER the service has been provided, especially for Emergency Room visits, and from referring outside of the Insurance Network for Routine Visits.
