Patient Statements
Once we hear back from your Insurance Company, depending upon your plan, a Patient Statement is sent for any CoPay, Deductible or Coinsurance which your insurance company has deemed owed to the physician.
Credit Card payments are accepted through the NFPA Patient Portal. Please review the Statement and your personal records to assure that a duplicate payment is not being made. Only the Last Statement is available to view on the NFPA Patient Portal and will remain viewable until a new statement is generated. Checks may be sent using the enclosed envelop, detaching the payment slip from the statement and sending to the address on the payment slip. |
