Tufts Health Plan Medicare Preferred

The Centers for Medicare Services allows for an additional layer of care via a health plan for Medicare members know as Medicare Advantage plans.
Needham Family Practice Associates has been a Provider of Tufts Health Plan Medicare Preferred (TMP) for many years. The Tufts Health Plan Medicare Preferred Program has received a Five Star rating from CMS for five years running. As a Provider of Tufts Health Plan Medicare Preferred (TMP) we meet multiple times each month with Case Managers and Tufts Health Plan Care Team to monitor and discuss TMP member's current treatment and care ranging from Home Health Care, Nursing Homes, Hospitalizations and finding the right Hospice Care for members in the final stage of life. This extra attention to your care allows NFPA and Tufts Health Plan Care Team to work together to find the best individual care and treatment plans for members.
